Middle Tennesee Mission Outreach requires that both a Liability Release Form and Team Member Information Form be completely filled out for each person attending a mission trip.  Simply filling the forms out does not guarantee you a spot on a particular trip, but If you are interested in attending a mission trip with MTMO, you will be required to print and fill out both the release and information forms.  The forms are included in links at the bottom of this page.  These forms can also be faxed, mailed or emailed to you.  Please contact Pat Harrell at pharrell@whud.org to have these forms forwarded to you or use another method on the Contact Us page to let us know where to send the forms.  Trip fees will not be accepted without these forms. Minors who are not traveling with all legal guardians must additionally fill out the Minor Travel Permission form and have it notarized.  See below for this form.

Only turn in the form at the following link if you are a minor that is not traveling with all your legal guardians.  Note this form must be notarized.  Send a copy to Pat Harrell at the contact information noted above and keep the original with you at all times on the trip.  If you are a minor traveling with all your legal guardians, you do not need to submit this form.

For children under 18 years of age, all parent or guardian signatures must appear on the release form.  For minors to travel with either a parent or another adult, a permission form must also be signed by all parents or guardians.  Children age 16 years and younger must be accompanied on MTMO mission trips by a parent or legal guardian - and have the minor permission to travel form completed as noted above.

A new release form and information form must be completed for each trip with MTMO.

If your contact information (name, address, phone number, email, etc.) changes after you send in your initial information form, it is your responsibility to contact MTMO with the updated information.  Inaccurate information may result in problems with airline tickets.  This could result in the airline not allowing you to board the plane at the last minute.  It is also very important for MTMO to have accurate records so we can keep you up to date with new information.

MTMO will also need a legible photocopy of your passport showing the photograph and information pages.   You can scan and email this to us or mail a copy.
